Revision discectomy and spinal fusion remain viable strategies for treating recurrent lumbar disc herniation, with no significant difference in 12-month Visual Analog Scale or Oswestry Disability Index outcomes between nonfusion and fusion methods.
Fusion may offer reduced recurrence in select cohorts but involves greater perioperative morbidity, including significantly higher blood loss.
Minimally invasive techniques (e.g.,) are associated with reduced complication profiles and higher patient satisfaction.
The effect of timing from symptom recurrence on short- and long-term outcomes remains poorly characterized due to inconsistent reporting, representing a key knowledge gap.

Domain 1: risk of bias arising from the randomization process; D’Oria et al. [19] (2023) (Allocation bias contributes to high risk for bias regarding randomization). Domain 2: risk of bias due to deviations from the intended interventions. Domain 3: risk of bias due to missing outcome data. Domain 4: risk of bias in measurement of the outcome; All (Lack of blinding for outcome assessors could lead to possible bias). a) Analysis of variance and chi-square testing found significant differences in gender distribution (p=0.034), smoking prevalence, alcohol consumption, diabetes mellitus, hypertension (p<0.001) across procedure groups; while age (p=0.166) and BMI (p=0.056) showed no significant variation.
b) Study inclusion criteria required at least 6 months of pain relief after primary lumbar disc surgery prior to symptomatic recurrence.
c) Study inclusion criteria required at least 1 month of pain relief after primary lumbar disc surgery prior to symptomatic recurrence.
d) Within Guan et al. [18] (2017) lumbar fusion group, patients underwent various types of surgery based on surgeon preference (n=7 by TLIF, n=3 by MIS-TLIF, and n=2 by PLIF).
